Companion Animals Frequently Asked Questions, Forms and Brochures

Companion animals (dogs/cats) become a part of your family and as such we want to ensure they are kept safe and are compliant with NSW legislation.  Please ensure you are familar with the NSW Companion Animals Act 1998.

Always keep your information up-to-date!

As a pet owner you must make sure your contact information as listed on the NSW Companion Animals Register is always up to date and current - if your pet goes missing you can't be contacted if your information is out of date.

How do I update my information?

There are forms that must be completed to update information:

 Change of Owner Details

If your dog/cat is being rehomed, hence a change in ownership, the 'Change of Owner / Details Form' MUST be completed and signed by both the old and the new owner.

It is the responsibility of the OLD OWNER to ensure the details are updated with council. This form is also to be completed if you are changing your name and/or address.

 Change of Address Notice

If you are only changing your address, you can use the 'Change of Address Notice' or the 'Change of Owner / Details Form' (above).

C3C - Change of Address Notice

 Verification of Existing Microchip

A 'Verification of Existing Microchip Form' is completed by an Authorised Identifier.  The Authorised Identifier must physically scan the dog/cat before completeting the form.

Where to send or take completed forms

All forms can be forwarded by mail or presented over the counter at your local council.  If you forward documentation by mail it is advised that by way of follow up you call council to ensure they have received the relevant paperwork.

Council is not responsible for documentation that has not been received via post.
