Wellness and reablement is now the expectation of practice in the Commonwealth Home Support Programme (CHSP).
Service providers are now required to report on their progress in implementing the approach in their funded programs.
• What are you planning to include in your wellness report?
• What evidence do you need to be gathering to demonstrate measureable outcomes for your clients from a wellness and reablement perspective?
• What else does your agency need to do to further implement the wellness and reablement approach?
These workshops will be a mixture of presentation and facilitated group discussion focussed on reporting requirements. You are encouraged to bring along your CHSP draft report to the session.

These workshops are designed for CHSP Managers, Coordinators and Team Leaders evaluating and reporting on Wellness and Reablement. 


Bookings essential. Limited places available. 

2 Sessions in One Day! Due to popular demand, and limited numbers per workshop, Glen will facilitate a morning and afternoon session.
Morning Session: 9:30am to 12:30pm
Afternoon Session: 1:30pm to 4:30pm
